During your first visit to Applied Chiropractic of Troy, please allow approximately one hour for the examination. Dr. Jessup will discuss the reason for your visit and learn about you and your medical history. She will then ask you to change into a gown. After completing the examination, Dr. Jessup will explain her findings and discuss your options for treatment.  Finally, with your approval and your shared commitment to success, you will be treated by the doctor. Subsequent visits will last approximately 30 minutes, where in most cases you may wear your street clothes instead of a gown.

Payment may be made by cash, check, or credit card and will be due at the time of your treatment. The clinic staff will provide you with the insurance codes appropriate to your diagnosis and treatment, allowing you to submit your claim to the insurance company for reimbursement.

Before attending your first visit, please download, print, and complete the Patient Intake Forms posted below – saving you time at our office. If, for whatever reason, you’re not able to complete these forms, please arrive 15 minutes prior to your appointment to obtain pre-printed copies.
